Registration for AORTIC 2009 is now closed. Delegate capacity has been reached and the AORTIC 2009 Conference Secretariat will accept no more registrations, either onsite or online.

No more registration forms will be accepted. Please do not intend to register onsite at the conference – registration is closed and no more bookings will be accepted.

Visa Required For Visiting Tanzania

All visitors to Tanzania require a visa in order to enter the country. The price is USD 50.00 (USD 200.00 for Pakistani visitors). Please visit the Tanzania Ministry of Home Affairs website here or download their "Visas Information" document here.

Provision has been made for AORTIC 2009 delegates to obtain a visa upon arrival in Tanzania. After you have registered and provided proof of payment, the AORTIC 2009 Conference Secretariat will issue a letter from the Tanzania Ministry of Home Affairs stating that AORTIC 2009 delegates may obtain their visa upon arrival. Please print out this letter, as well as your letter of confirmation from the AORTIC 2009 Conference Secretariat, and present it with your passport in order to facilitate the visa process.

PLEASE NOTE:

  • All visitors to Tanzania require a visa and it is your responsibility to carry enough cash to pay for this visa and to ensure that your travel document (passport) is valid.
  • Visas are not included in bursary support.
  • The AORTIC 2009 Conference Secretariat will try to answer any visa-related questions, but please note that the Secretariat is not responsible for delegates' visa applications.

AORTIC 2009 Accreditation:

AORTIC 2009 has been accredited by the University of Cape Town (UCT), Faculty of Health Sciences Accreditation Committee and registered for 30 CPD points. In order to qualify for these points, please be advised that you will need to obtain a Certificate of Attendance from the AORTIC Registration Desk, as well as sign the register for all conference days.

AORTIC 2009 Registration Terms and Conditions

1. Registration will only be valid once all monies are received.
2. All payments to be made payable to AGENDA DESTINATION MANAGEMENT.
3. All cancellations must be received in writing.
- Cancellations 90 days prior to the beginning of the conference will receive a full refund less a 10% administration fee.
- Cancellations between 90 and 30 days will be refunded 50% of the fees less the administration fees; if registration fees are outstanding, the delegate will be liable for 50% of the conference fees.
- No refunds will be provided for cancellations or no-shows within 30 days of the start of the conference and the delegate will be liable for the full conference fee.
4. Completed registrations will be acknowledged within 7 days.
5. Registrations are transferable to delegates not yet registered only if the Conference Secretariat is informed in writing.
6. All fees are payable in US Dollars (USD). South African delegates may pay in South African Rand (ZAR) on request. Any short payment resulting from bank charges and/or exchange rate fluctuations will be charged back to the delegate.

AORTIC 2009 Accommodation

Dar es Salaam has a wide range of accommodation options to suit all tastes and budgets. The AORTIC 2009 Conference Secretariat has selected a range of hotels for delegates to choose from and has negotiated guaranteed rates. Delegates can reserve accommodation at one of the designated conference hotels through the registration process.

AORTIC 2009 Accommodation Terms and Conditions

HOTEL RESERVATION PROCEDURE
1. Reservations will be made on behalf of delegates by the AORTIC 2009 Conference Secretariat. Please complete the appropriate section on the registration form specifying single or double occupancy, check-in/ check-out dates, and any other special requests.
2. To ensure that your accommodation is secure, please book prior to 10 August 2009. Any accommodation requests received after this date will be dealt with on a per availability basis.
3. In order to secure your accommodation, the full amount due is required. Please ensure that your credit card is valid through the end of your stay, failing which the booking may not be guaranteed. Any extras must be settled in US Dollars (USD) upon check-out.
4. Cancellations 60-30 days from arrival date, a 50% penalty will be charged. For cancellations within 30 days of the arrival date or failure to arrive at all (no-shows), a 100% penalty will be charged. Should you fail to arrive as scheduled, the hotel will charge for the loss of revenue for the night or nights in question.

All reservations are made on a 'first come, first served' basis. The stated room rates and availability are valid through 10 August 2009. Thereafter, no rates or availability can be guaranteed and requests will be dealt with according to the current availability and rates at the hotels.
